Having given birth to a baby boy, Louisa returns to Portwenn and Martin decides to delay his move to London by a day or so. Louisa is exhausted most of the time and Martin does his best to help her carry the load. He's not impressed with his replacement as the village doctor, Diana Dibbs, who seems to have several medical ailments of her own to take care of. When he sees the types of prescriptions she seems to hand out to everyone who visits her, he begins to question her competence. Martin receives some bad news when he's told his Aunt Joan has been in a car accident.
